To Superintendents and Principals:
The Mississippi State Department of Health 2025-26 Preliminary Compliance Report Form is now available via Smartsheet. The assessment should be completed and submitted by Sept. 19, 2025.
It is the responsibility of the school administrators to determine which children have no compliance forms or have an incomplete form.
Please note the changes made to this year’s assessment found in the new 2025-26 Facts Sheet.

Please read the attachments thoroughly as they provide detailed information to assist you in completing the assessment:

A hard copy of this letter will be sent via USPS. Please only submit your Preliminary Assessment once. If for any reason you need to correct information that is already submitted, please contact our office to assist.
ALL COMPLIANCE REPORTS MUST BE COMPLETED ELECTRONICALLY AND SUBMITTED VIA THE SMARTSHEET ASSESSMENT FORM PORTAL.
